About the role

Provide advanced primary and acute care by performing comprehensive assessments, diagnoses, and treatment plans for patients. Collaborate with interdisciplinary teams to coordinate multidisciplinary care, referrals, and patient education.

Job Summary Provide advanced primary and acute care to patients across health care settings.

Perform comprehensive assessment, diagnosis, treatment, and health promotion in collaboration with interdisciplinary teams.

Number of Positions 5 Main Duties Perform advanced health assessments and clinical examinations to diagnose acute and chronic conditions.

Develop, implement, and evaluate individualized treatment and care plans, including prescribing medications and ordering diagnostic tests within scope of practice.

Manage patient care across settings, including primary care, urgent care, and specialty clinics, and provide follow-up and continuity of care.

Collaborate with physicians, nurses, and allied health professionals to coordinate multidisciplinary care and referrals.

Provide patient and family education, health promotion, and disease prevention advice tailored to patient needs.

Maintain accurate clinical documentation, adhere to professional standards, and participate in quality improvement and clinical governance activities.

Requirements

Completion of an accredited graduate nurse practitioner program or equivalent advanced nursing education. Current registration or licensure as a nurse practitioner with the appropriate provincial or territorial regulatory authority. Authorization to prescribe medications and order diagnostic tests within the jurisdictional scope of practice. Minimum of 2 years of recent clinical nursing experience, with preference for primary care or relevant specialty experience. Current Basic Life Support (BLS) or equivalent certification and willingness to maintain required clinical certifications. Clear criminal record and ability to meet employer-required regulatory and credentialing checks. Required Skills Advanced clinical assessment and diagnostic reasoning. Pharmacotherapy knowledge and safe prescribing practices. Clinical decision-making and independent practice management. Effective interpersonal and interdisciplinary communication. Proficiency with electronic medical records and clinical documentation. Patient education, counseling, and health promotion skills.
