About the role

Provide evidence-based nutrition assessments and counseling to individuals and groups to improve health outcomes. Collaborate with interdisciplinary teams to develop, implement, and monitor individualized nutrition care plans.

Job Summary Provide evidence-based nutrition assessment and counseling to individuals and groups to support health outcomes.

Collaborate with interdisciplinary teams to develop and monitor nutrition care plans.

Number of Positions 2 Main Duties Conduct nutritional assessments using medical history, dietary intake, anthropometric measurements and relevant clinical data.

Develop and implement individualized nutrition care plans and therapeutic diet recommendations.

Provide one-on-one and group nutrition counseling and education for clients, families and caregivers.

Monitor client progress, evaluate outcomes and adjust interventions based on clinical indicators.

Collaborate with physicians, nurses and other health professionals to integrate nutrition into overall care plans.

Document client interactions, prepare reports and contribute to program development, quality improvement and community outreach.

Requirements

Bachelor's degree in dietetics, nutrition science or a closely related field. Completion of an accredited practicum or internship in dietetics or equivalent supervised practical experience. Registration or eligibility for registration with the provincial regulatory body for dietitians (or equivalent) as required by jurisdiction. Experience conducting nutritional assessments and developing care plans in clinical, community or public health settings. Ability to work collaboratively in interdisciplinary teams and manage multiple caseloads effectively. Proficiency with electronic health records, nutrition analysis tools and common office software. Required Skills Nutritional assessment and individualized care planning Individual and group counseling and client education Clinical decision making and critical thinking Interdisciplinary collaboration and teamwork Accurate documentation and reporting with attention to confidentiality Use of electronic health records and nutrition analysis software
