Clinician (GAIN)

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.

With a staff of more than 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are “home-bound” and frail.

We are currently seeking a GAIN Clinician to fill a vacant position at our Geriatric Assessment & Intervention Network (GAIN) Team.

As a member of the interprofessional Geriatric Assessment and Intervention Network (GAIN) team, the GAIN Clinician supports older adults with complex medical, cognitive, functional and psychosocial needs. The clinician contributes profession-specific knowledge and interventions to comprehensive geriatric assessment, collaborative care planning, follow-up and care coordination. All assessments, interventions, documentation, delegation and referrals are completed within the successful candidate's legislated scope of practice, competence, professional standards and Carefirst policies.

Terms: Full Time/ Part Time Contract (Until September 10, 2027)

Location

300 Silver Star Blvd., Scarborough and community locations in the GTA (including office-based work, computer use, community outreach and home visits)

Pay range

$55,000 - $85,000 annually

Responsibilities

• Assessment and care planning: Conducts discipline-appropriate assessments and contributes to comprehensive geriatric assessments, including review of functional status, mobility and falls risk, cognition, mood, medication-related or health concerns, social determinants of health, caregiver needs, home safety, ADLs/IADLs and community supports, as applicable to professional scope and competence
• Intervention and follow-up: Develops, implements, monitors and evaluates individualized, person-centred plans in collaboration with the client, substitute decision-maker where applicable, caregivers and the GAIN team. Provides education, counselling, clinical or functional interventions, navigation, referrals and follow-up within scope
• Collaboration and transitions: Works with primary care, hospitals, Ontario Health atHome, community agencies and other partners to coordinate services, reduce gaps in care, support safe transitions and promote aging in place.
• Documentation - Documents assessments, interventions, consent, care plans and outcomes accurately and promptly in the electronic medical record. Participates in case conferences, program evaluation, quality improvement, education and required training.
• Safety and professional accountability - Follows applicable legislation, regulatory standards, privacy and consent requirements, infection prevention and control practices, workplace violence and harassment procedures, safe community/home-visit practices and incident-reporting requirements. Identifies and escalates safety concerns in accordance with Carefirst policies
• Other duties as assigned in support of operations

Qualifications

• Degree, diploma or other credential required for registration in an eligible Ontario regulated health or social service profession. Current registration in good standing with the applicable Ontario regulatory college, with no restrictions that prevent performance of the essential duties. Eligible backgrounds may include, but are not limited to: Registered Nurse, Registered Practical Nurse, Occupational Therapist, Physiotherapist, Social Worker, Kinesiologist, Dietitian, Pharmacist, Speech-Language Pathologist or another regulated professional whose authorized scope and competencies align with the position.
• Experience working with older adults in community, primary care, hospital, rehabilitation, geriatric outreach, mental health or related settings is preferred. Gerontology or geriatrics experience is an asset.
• Knowledge of person-centred geriatric care, frailty, dementia and cognitive impairment, responsive behaviours, falls prevention, caregiver support, community resources and social determinants of health. Ability to assess risk, exercise sound professional judgment, organize a complex caseload, work independently and collaborate effectively on an interprofessional team
• Strong written and verbal communication skills in English, at the level required to provide safe care and maintain clinical documentation. Additional language skills relevant to communities served, including Cantonese or Mandarin, are an asset. Proficiency with electronic medical records and Microsoft Office applications is required.
• Training in geriatrics, dementia care, GPA, PIECES, motivational interviewing, falls prevention, mental health, behaviour support or care navigation.
• Current First Aid/CPR certification may be required based on assigned functions.
• Regular travel to Carefirst offices, community locations and client homes in the service area is an essential duty. Must be able to travel reliably between assigned locations
• A valid vulnerable sector police record check completed within one year
