Develop and manage individualized treatment plans, including medication management and therapeutic interventions.
Collaborate with the interdisciplinary PACE team, including physicians, nurses, social workers, therapists, and the collaborating psychiatrist, to ensure coordinated participant care.
Provide ongoing psychiatric follow-up, crisis intervention, and behavioral health support across care settings.
Educate participants and caregivers on mental health conditions, medications, and treatment options.
Maintain timely, accurate documentation and participate in quality improvement initiatives to enhance behavioral health services within the PACE program.
Master's or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health Current and unrestricted PMHNP license in CA Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) Minimum of 1-year clinical experience as a PMHNP, preferably in a hospital or acute care setting Strong knowledge of psychiatric assessment, diagnosis, and treatment Active and unrestricted DEA (Drug Enforcement Administration) (We will help obtain) Excellent communication and interpersonal skills Ability to work collaboratively in a multidisciplinary team Psychiatric Nurse Practitioners will maintain clinical competency & collaboration with Psychiatrist (we will provide)
Requires a Master's or Doctoral degree in Nursing specializing in Psychiatric Mental Health and a current unrestricted PMHNP license in California. Candidates must be bilingual in Spanish and have at least one year of clinical experience, preferably in acute care.
