About the role

Responsible for nutritional assessment, care plan implementation, and evaluation for acute and subacute adult patients at the Reactivation Care Centre. Coordinates nutrition care services to facilitate patient transitions from hospital to home and contributes to quality improvement.

Who you are: You encompass Oak Valley Health’s core values and live the words of respect, trust, commitment, compassion and courage You are a patient and/or client centered individual who performs at an exemplary standard You are a team player with excellent communication, critical thinking and prioritization skills What we are looking for: As a member of our interprofessional team, you will be responsible for the nutritional assessment, care plan implementation and evaluation for a variety of acute and subacute adult patient populations at Reactivation Care Centre site .

You will utilize your sound knowledge in clinical dietetics to meet patient/family needs while delivering the highest standard of evidence-based care.

Your excellent clinical leadership skills will assist you in working with both the interprofessional and Clinical Dietitian teams.

You will utilize your excellent knowledge of best-practices in nutrition care to provide acute nutrition assessment, treatment, coordination of nutrition care services to bridge your patient's transition from hospital to home and effectively contribute to quality improvement.

What you bring to the role

Current membership in the College of Dietitians of Ontario, in good standing Membership with Dietitians of Canada (DC), preferred BLS Certification is required CNSC designation is an asset A minimum of 2 years clinical experience in nutrition care support of adult patient population in an acute care setting Demonstrated commitment to ongoing professional development and evidence-based practice Excellent assessment, organizational, and interpersonal skills. Demonstrated excellence in verbal and written communication skills Demonstrated competency with EMR charting systems, CBORD, and associated computer skills Ability to work independently and as part of a interprofessional team Access to own transportation to travel in between sites Demonstrated good attendance and performance records with the ability to maintain these same standards Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

Work

Schedule: 8 hr shifts (0830-1630) every Wednesday and every other Friday (flexible with second day) at RCC site Please note schedules may change due to operational needs.

Compensation

  • Range: $42.55 - $54.18

Requirements

Requires current membership in the College of Dietitians of Ontario and a minimum of 2 years of clinical experience in acute care for adults. BLS certification is required, while CNSC designation and Dietitians of Canada membership are preferred.
