EOD August 17, 2026 What we are looking for: As a member of the Reactivation Care Centre team, you will work within a patient-focused care model in the provision of quality, customer focused, patient/family centred care. You will utilize your demonstrated clinical leadership, case management, analytical, problem solving and interpersonal skills to develop, implement and evaluate comprehensive plans of care to meet the patient/family needs. Your excellent clinical leadership skills will assist you in working with the interprofessional team in the further development of programs to enhance and support patient care.
