Providence Health Care is hiring for a Regular Part-Time Social Worker position in the Road to Recovery Program.Join our innovative, interdisciplinary team providing holistic, person-centred, trauma-informed care.

With the new stabilization unit that opened in Fall 2023 and the continued expansion of the program, you'll be part of a leading approach to recovery.

Our program offers detox beds, transitional care, and aftercare outreach services, ensuring clients receive the right support at every stage of their recovery journey.

In accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, the person promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.

Reports to the Professional Practice Leader and works under the supervision of the designated Site Leader.

Collaborates with Site Leaders and Professional Practice Leaders across the interdisciplinary program team.

Works as part of an interprofessional team to provide therapeutic clinical counselling to individuals and families affected by substance use.

Conducts comprehensive biopsychosocial assessments.

Develops, implements, and evaluates individualized treatment plans.

Provides case consultation and follow-up support related to treatment decisions in collaboration with the interprofessional team.

Supports multidisciplinary care teams in addressing complex care needs.

Participates in discharge planning and helps determine the effective use of available resources.

Contributes to the development and coordination of community resources.

Liaises with Primary Health Care Addiction and Mental Health teams.

Participates in continuous quality improvement initiatives that promote excellence in social work practice.

Contributes to approved social work and interdisciplinary research initiatives and projects.

Participates in committees and group meetings that help connect hospital programs with community-based supports and services.

What You Bring Education, Training and Experience: Master’s Degree in Social Work from an accredited university.

Postgraduate continuing education in model(s) of therapeutic intervention in addictions and mental health is preferred.

One year of recent related experience working in the areas of addiction, mental health, HIV/AIDS and complex pain, or an equivalent combination of education, training and experience.

Current full registration with the British Columbia College of Social Workers.

A Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training and experience, may be considered.

Current full registration with the British Columbia College of Social Workers.

Skills and Abilities: Demonstrated knowledge of drug and alcohol addiction issues and related evidence-based best treatment practices.

Comprehensive knowledge of psychiatric diagnoses and professional practice guidelines for the treatment of various disorders, including complex concurrent disorders.

Demonstrated ability to communicate and work effectively with clients, families, interprofessional teams and community partners.

Demonstrated understanding of the social determinants of health.\ Demonstrated ability to conduct comprehensive psychosocial assessments that include standardized mental health and addictions frameworks and tools.

Ability to plan, implement and evaluate client interventions based on psychosocial assessments.

Ability to make appropriate and effective referrals that reflect knowledge of community resources.

Demonstrated skills and ability to apply theory to practice in the areas of case management, resource and supportive counselling, and individual and group therapy for clients and families coping with problematic alcohol and drug use.

Knowledge of the scope of practice of other health care disciplines and the ability to work independently and collaboratively within an interprofessional team.

Ability to work effectively within a culturally diverse environment.

Demonstrated ability to organize and prioritize workload in a complex and demanding environment.

Demonstrated comprehensive knowledge of regulations, policies, procedures and other statutory duties related to relevant legislation, such as the Mental Health Act, Adult Guardianship Act and Freedom of Information and Protection of Privacy Act.

Ability to educate and provide consultation to clients, families and health care professionals, including the development, coordination and delivery of in-services, presentations, psychoeducational groups and support groups.

Ability to participate in research.

Proficiency in the use of personal computers, including related word-processing and database-management software applications.

Physical ability to carry out the duties of the position.

Your Day-to-Day: Provide therapeutic clinical counselling to clients, families and groups, including facilitating behavioural change, coordinating services and delivering therapeutic group programs. Conduct comprehensive psychosocial assessments by gathering and interpreting information about clients’ mental health, substance use, family supports, culture, living situation and financial circumstances. Develop, implement and evaluate individualized care and treatment plans in collaboration with clients and interprofessional teams across inpatient and outpatient settings. Connect clients and families with appropriate community resources, treatment programs and rehabilitation services through referrals, advocacy and consultation. Maintain accurate electronic client records, including assessments, progress notes, care plans and reports, in accordance with professional, regulatory and organizational standards. Provide clinical guidance, education and consultation to social work colleagues, health care professionals, clients, families, students and community partners. Participate in the development, delivery and evaluation of addiction and mental health programs, staff education, presentations and professional-development initiatives. Collect and analyze workload and program data, identify trends and recommend improvements to support effective service delivery. Participate in approved research and special projects by reviewing evidence, developing proposals and protocols, conducting research activities and sharing findings. Perform other related duties as assigned. Requirements

Requires a Master's or Bachelor's degree in Social Work and current full registration with the British Columbia College of Social Workers. Candidates should have at least one year of related experience in addiction, mental health, or complex pain management.
