The RN Case Manager will report to the Primary Health Nursing Coordinator and Manager of Primary Health Care. This position will be an integral part of the Primary Care Health Team within the Pinehouse Health Centre. Although the primary base of work will be in a primary care setting, some community outreach and programming will be required. The RN Case Manager will also provide support in areas of public health and communicable disease management. The focus of the RN Case Manager will be to offer patient case management through holistic care coordination within a multidisciplinary health care team. The RN Case Manager will take a lead in fostering close therapeutic relationships between the primary care customer, their families, and the wider clinical team primarily through disease management and acute conditions which involve assessment, diagnosis, referral, health teaching, advocacy, and health promotion. There will be limited transfer of medical function (ToMF), which will advance to the Authorized Specialty Practice through expanded training as set out by the College of Registered Nurses of Saskatchewan (CRNS). The RN Case Manager will be working to their full scope of practice as established by the CRNS.

Requires a Bachelor's Degree or Diploma in Nursing or Psychiatric Nursing and a minimum of two years of healthcare facility experience. Candidates must be registered or eligible for registration with the CRNS or CRPNS and hold ACLS, PALS, and BCLS certifications.
