Description de poste : Relevant de la gestionnaire des services sociaux, de la psychologie et de la nutrition, le travailleur social sera responsable de l’évaluation psychosociale et de l’identification des besoins psychosociaux et financiers des patients et de leur famille. Nous adoptons une approche systémique dans notre travail et nous nous engageons à fournir la meilleure qualité de soins aux enfants et à leurs familles.
Principales responsabilités : Évaluer et identifier les besoins sociaux et financiers des familles Accompagner les familles plus vulnérables lors de leur cheminement au Shriners (du premier rendez-vous en clinique ambulatoire, à l’après chirurgie) ; Rédiger le plan de traitement psychosocial en tenant compte des ressources de la famille et du milieu de vie ; Représenter les familles auprès de l'équipe médicale et interdisciplinaire ; Favoriser la communication mutuelle entre l'équipe médicale et la famille ; Référer et orienter les familles vers les ressources du milieu et faire le lien avec les professionnels déjà impliqués ;
Intervenir en situation de crise : Accompagner les parents dans leurs questionnements par rapport à leur rôle parental ; Faire le lien avec l’équipe transport hébergement ; Explorer des pistes de solutions et orienter les familles vers les ressources appropriées : S’assurer du respect des différentes réalités culturelles et linguistique et des droits des familles aidées ; Participer à la recherche clinique, à divers comités et à des projets d'amélioration de la qualité.
Qualifications requises : Doit détenir un diplôme universitaire en travail social Membre de l’Ordre professionnel des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec Minimum d’un (1) an d’expérience en travail social Une expérience dans le réseau de la santé constitue un atout Excellente capacité d’écoute et d’entraide Bonne capacité à entrer en relation avec autrui Joueur d’équipe, passionné et collaborateur Sens de l’organisation, initiative, autonomie Solides aptitudes pour la communication, les relations interpersonnelles et l'analyse

Job summary: Reporting to the manager of social services, psychology, and nutrition, the social worker will be responsible for the psychosocial assessment and identification of the psychosocial and financial needs of patients and their families. Key responsibilities : Assess and identify the social and financial needs of families Support more vulnerable families during their journey at Shriners (from the first outpatient clinic appointment to post-surgery); Draft the psychosocial treatment plan taking into account the family's resources and living environment; Represent families to the medical and interdisciplinary team; Promote mutual communication between the medical team and the family; Refer and guide families to community resources and connect with professionals already involved; Intervene in crisis situations; Support parents in their questions regarding their parental role; Liaise with the transport and accommodation team; Explore possible solutions and direct families to appropriate resources; Ensure respect for different cultural and linguistic realities and the rights of assisted families; Participate in clinical research, various committees, and quality improvement projects.
