Lahey Clinic Lung Health Team, APP Bronchiectasis Program Development (Deployed to Beverly Hospital) Location: Burlington, MA (Lahey Hospital & Medical Center) Type: Full-time | 2-Year Grant-Supported (potential for extension) Schedule: Weekday outpatient The Opportunity Lahey Hospital & Medical Center is seeking an innovative NP or PA to help launch a novel, population-based Bronchiectasis Program. This unique role sits at the exciting intersection of technology, population health, and clinical care. You will use Natural Language Processing (NLP) screening of radiology reports to identify at-risk patients, drive structured workflows, and deliver guideline-aligned clinical care. Time Allocation 50% Patient Navigation & Care Coordination (Chart reviews, registry tracking, scheduling, patient outreach). 50% Direct Clinical Care (In-person outpatient visits and longitudinal management). Key Responsibilities Patient Navigation & Operations (~50%) Screen & Review: Evaluate NLP-flagged cases via structured EHR chart reviews to confirm program eligibility.
Engage primary care teams and contact patients via phone, letters, and MyChart.
Coordinate
Care: Facilitate diagnostic pathways (PFTs, sputum cultures, labs, imaging) and ensure timely pulmonology scheduling.
Perform risk stratification, symptom reviews, and develop evidence-based care plans under physician supervision.
Support multidisciplinary case reviews and participate in Lung Health Team meetings.
Qualifications
Required: Licensed Nurse Practitioner or Physician Assistant eligible to practice in MA. Strong organizational skills with comfort balancing clinical care and outreach. Excellent communication skills with patients and multidisciplinary stakeholders. Proficiency with EHR workflows.
