In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. Reporting to the Coordinator, Cowichan Lodge - Adult Tertiary and working as part of a multidisciplinary team, the Social Worker provides assessment, treatment, case management and mental health support services for clients. Responsibilities include conducting assessments and providing recommendations for clients' continuum of care for successful transition through program options, educating others regarding mental health issues and participating in staff education regarding government programs, community agencies and services/resources available for people with mental health issues. Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle.
