Assess client needs and facilitate access to second opinion consults and treatment plans. Provide health coaching using motivational interviewing and maintain accurate electronic patient records.
Requires an RN degree from an accredited school, provincial registration, and CPR certification. A mental health certification with at least two years of clinical experience is preferred.
RN level nurses who have graduated from an accredited School of Nursing and hold a current registration with the provincial licensing body. mental health certification with minimum two years demonstrated clinical experience in a mental health setting, managing patients with both acute and chronic medical conditions preferred Current CPR certification required
Assess the client’s specific program needs Facilitate access to provider of second opinion consults: Collect necessary information (condition specific questionnaires) and consents Support collection of medical records required for the second opinion Arrange for and participate in the post second opinion conference with the client and their families to discuss the results Provide an explanation of the recommended treatment plan options as required Facilitate the implementation of the recommendations from the conference Assist with any follow-up questions back to the second opinion provider Assist with setting up any treatment options. Utilizing standard tools, assess the clients need for health coaching considering the individuals knowledge of the condition, stability of the condition and/or motivation to self-manage their condition Provide health coaching using motivational interviewing techniques to assist clients with goal setting based on strategies developed for the individuals readiness to learn Assess clients health coaching outcomes using standardized tools Provide strategies to reduce side effects of medications and treatments Guide the client too alternate treatment location(s) if requested Document all client related activities (tasks, phone calls and emails) in the client electronic database, including duration of these activities Ensure patient records are maintained and data accurate to meet program reporting requirements Participate in program quality activities and continuous improvement initiatives in keeping with the company’s Quality Management System and ensuring best practice is reflected Maintaining skills and competencies by participating in ongoing internal and/or external continuing education activities Adhere to Bayshore Policies and Procedures Participates in proactive Health and Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health and Safety risks or concerns. Responsible for completing Accident Reports for all direct reports who injure themselves on the job, within 24 hours of the incident Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel Complete other tasks as requested Other Requirements: Clear criminal background check (with vulnerable sector screening), negative TB test Valid driver’s license, auto insurance and reliable vehicle Excellent command of the English language; spoken and written Motivational interviewing skills/experience an asset Strong critical thinking and problem solving skills Able to work well with team members internal and external to the organization Required to work independently Requires advanced knowledge of computer programs (Microsoft Word, Excel, Outlook, PowerPoint) Mindful of accuracy and thoroughness of data entry for reporting requirements Excellent telephone and customer service skills Excellent written and verbal communication skills that include active listening At Bayshore Healthcare, we are committed to fair, equitable, and transparent compensation practices.
