About the role

Provide high-quality primary medical care to individuals experiencing homelessness across various clinics and outreach programs. Collaborate with a multidisciplinary healthcare team to ensure comprehensive care using trauma-informed and harm reduction principles.

This position will be posted for a minimum of 5 days The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado.

The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

This shift for this position will be Monday thru Friday 7

  • 00 am - 5:00 pm 3 days and 8:00 am - 6:00 pm on Wednesdays. You can select Tuesday or Thursday as your day off. The newest Primary Care Physician at the Colorado Coalition for the Homeless (CCH) will join a group of experienced, dedicated, and compassionate medical providers to provide high-quality primary care to some of the most vulnerable patients in the Denver metro area. This physician will care for individuals currently or formerly experiencing homelessness in fast-paced, exciting, and varied environments. No two days are the same. This position will serve across various programs including the Stout Street Health Center, the Recuperative Care Center, Street Medicine, home visits, Health Outreach Program (mobile clinic), and satellite clinics. The physician will deliver care using the principles of motivational interviewing, harm reduction, and trauma-Informed care to support patients in achieving better health outcomes.
  • NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete. \n

Additional Requirement Valid driver’s license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business and/or role.

  • Essential Job Functions Provide direct, high-quality primary medical care to people experiencing homelessness.
  • Exhibit a positive attitude and a commitment to social justice in all aspects of patient care.
  • Utilize motivational interviewing, harm reduction, and trauma-informed care principles when working with patients.
  • Collaborate effectively with other primary care medical providers as well as the multidisciplinary healthcare team (psychiatry, behavioral health, case management, dental, vision) to ensure comprehensive care.
  • Perform other job duties as assigned.

Qualifications

Summary Demonstrated experience caring for patients experiencing homelessness, substance use disorders, poverty and/or mental illness. Current licensure or ability to obtain licensure in the State of Colorado as a physician. Current DEA licensure. Education Medical Degree or Doctor of Osteopathy Degree from an accredited school of medicine. Completion of an accredited Residency Program in the physician's specialty. Board Certified or Board Eligible status in the physician's speciality.

Requirements

Requires a Medical Degree or Doctor of Osteopathy, completion of an accredited residency, and board certification or eligibility. Must possess a current DEA license and the ability to obtain licensure in the State of Colorado.
