The Systems Navigation Support Worker will assist clients in navigating the healthcare system and connecting them with necessary services. They will work collaboratively with a multidisciplinary team to implement individual service plans and provide outreach to clients in various settings.
Candidates should have at least two years of experience working with individuals facing mental health issues or health inequities. A valid driver's license and relevant education in human services or related fields are required.

Position Overview My Health Team is a collaborative partnership between independent primary care practices, community organizations and regional health authorities. Partners work closely with one another to plan, develop and provide enhanced local primary care services to their client populations within their geographic area. This also will support the client journey through the health system and across community and acute care. The Systems Navigation Support Worker will provide proper linkage for clients of a My Health Team network of primary care providers, where services needed are not available through existing services, providers or where these are not accessible within the required timeframes throughout the healthcare system.
Through collaboration and partnership across various health, social service and community sectors, the program strives to promote mental health and well-being for all, to reduce inequities and their impact on vulnerable populations, and to work towards sustainability of mentally healthy individuals and communities.
The primary focus of the My Health Team is on enhanced primary care service through integration of healthcare services on health promotion, disease prevention, chronic disease self-management, and linkage with community-based resources. As a member of a multidisciplinary team the Systems Navigation Support Worker will assist in carrying out individual service plans under the direction of Primary care provider. The Systems Navigation Support Worker is responsible for rehabilitative and supportive functions including skill building/enhancement; creation and maintenance of formal and informal support networks; linking to and maintaining formal and informal support services; provision of appropriate and accurate information and consultation with families as related to skill building/enhancement and development of supports; community needs; identifying clients at risk for illness; strengthening and increasing supportive referral networks; and identifying linkages to community-based resources to assist all Network clients in navigating the service systems.

Experience with population who faces health inequities and challenges with determinants of health. Experience/education with de-escalation techniques. Experience in life skill teaching and support to individuals who struggle with mental health issues or other disabilities or conditions. Experience in developing, planning and coordinating services.
Education (Degree/Diploma/Certificate) University or College education in human services, other related disciplines, or high school diploma and related training/experience.
Certification/Licensure/Registration Requires a valid driver’s license and a vehicle.

Able to use problem resolution skills to handle sensitive and complex issues related to multiple service involvement and interdisciplinary teams. Knowledge of community health and support services. Knowledge of immigrant and Indigenous issues and cultural issues an asset. Demonstrated skills and competence to establish supportive, trusting relationships with individuals, and other stakeholders. Demonstrate flexibility, creativity and be an enthusiastic team member. Excellent communication and interpersonal skills. Knowledge of Recovery as it applies to individuals with mental illness is an asset. Accuro knowledge an asset. Computer literacy.
Physical Requirements Evening and weekend work will be required.
