Provide psychosocial screening, crisis intervention, and bereavement counselling for patients in Obstetrics, Gynecology, and Children's Hospital programs. Act as a liaison to community agencies and provide support for adoption, surrogacy, and uninsured patients within an interdisciplinary team.
Requires a Master's Degree in Clinical Social Work and registration with the Ontario College of Social Workers and Social Service Workers. Candidates need at least 3 years of experience with children and families in a tertiary care pediatric setting.
This position involves working as part of a team of social workers providing inpatient and outpatient care within both the Obstetrics/Gynecolgy and Children%27s Hospital Programs. Patient care areas include: Labour and Delivery, Ward 4C (postpartum), 4B1 Clinic (high risk obstetrics and gynecology), 2F Clinic (Prenatal Diagnosis and general obstetrics), Ward 4B (antepartum/postpartum and gynecology), Women%27s ICU, Neonatal Intensive Care Unit, and Intermediate Care Nursery. As a member of the interdisciplinary health care team, the Social Worker provides services including psychosocial screening and assessment; crisis intervention; bereavement counselling; identification of child welfare issues; liaison and referral to community agencies; adoption support; surrogacy support; support for uninsured patients; consultation; advocacy and education.
