The Primary Health Care Nurse provides and coordinates comprehensive nursing services to patients within primary health care sites. They are responsible for promoting quality health outcomes through case management and participating in community development initiatives.
Candidates must hold a Bachelor's Degree or Diploma in Nursing and a Basic Life Support Certificate. Preference is given to those with two years of community-based nursing experience and a background in case management.

Reporting to the Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The Primary Health Care Nurse promotes quality health care outcomes working within established standards for case management practice. The Primary Health Care Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.
