Responsible for monitoring and supporting clients transitioning from the hospital to the community. Duties include assisting with housekeeping, recreational activities, and social engagement under client direction.
Requires some high school education and the ability to empathize with people with physical and emotional needs. Prior experience with disabilities or seniors and first aid certification are preferred assets.
Under the direction of the Program Coordinator, this position is responsible for regular monitoring and provision of support to clients in the transition from hospital to the community. Duties include monitoring, observing, assisting with and participating in housekeeping, recreational and social activities. Such duties are carried out under the direction of the client or alternate decision maker and in accordance with Client Service Contracts, laws and regulations governing service provision. Employment Guide: · Some high school education · Prior experience or knowledge of providing support services for people with disabilities and/or seniors an asset. · Certification in health and safety and first aid preferred. · Ability to empathize with people with physical and emotional needs is essential. · Knowledge of basic math and ability to do calculations and quantity measurements are required. · Must be able to plan and organize work and perform tasks according to priority. · Ability to develop supportive relationships and work collaboratively with clients, family members, colleagues, volunteers, and community partners are essential. · Must be able to take direction and work independently as well as part of a team.
