Requires a Bachelor's degree in Social Work or a related discipline and at least three years of experience in community outreach or intensive case management. Must possess a valid G Driver's License and a reliable vehicle.

Developmental Services - Housing Stability Case Manager (Prevention & Shelter Diversion)
One Full-Time Contract to March 2029 (with the possibility of extension or becoming permanent) Location: Aurora, Ontario L4G3S9 must be within a commutable daily distance from the office and/or service area Status: Contract, Full-Time Starting
$32.13 / Hour Office Hours: Monday - Friday - 8:30 am - 4:30 pm, 35 Hours per week Office Expectations; Work in the office to provide team coverage, minimum once per month in addition to in-office meetings, trainings, supervisions and all-staff events Posting Closes: Friday, June 12 at 4:00 pm EST
The Housing Stability Outreach Worker provides front-line homelessness prevention and shelter diversion supports for people in York Region who are experiencing homelessness or are at imminent risk of losing housing (within 30 days). The role delivers mobile, community-based outreach; completes housing stabilization and diversion planning; coordinates short-term emergency assistance; and connects people to crisis services, mental health supports, developmental services, income supports, and other community resources to support sustainable housing outcomes.
Successful candidates will be required to submit proof of a primary series of a COVID-19 vaccine authorized by the World Health Organization directly to the Human Resources department. In the event that you are unable to be vaccinated as a result of a ground protected under the Human Rights Code, you may submit a written explanation of the ground and any supporting documentation to determine if you are exempt from this requirement directly to Human Resources.
