This position provides intensive case management and advocacy services to individuals aged sixteen and over experiencing mental health, dual diagnosis, or addiction challenges. The role involves direct service interventions, clinical support, and community development to facilitate recovery and improve quality of life.
Candidates must hold a bachelor's degree in social work or a related discipline and possess at least three years of experience in social services. A valid G driver's license and access to a reliable vehicle are required for daily travel within the York region.

This position will provide intensive case management and advocacy services to individual’s aged sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless. Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life. Tasks will include direct service interventions, clinical support and community development. Daily travel within York region will be required.
