Position Summary

Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within an existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner, helping clients to connect with external resources, build new skills and achieve their goals.

Qualifications

EducationMust have a Valid Drivers’ License and access to a reliable vehicle. Skills, Expertise and Experience Minimum of 2 years’ experience working in community, preferably with program target populationExperience providing outreach with hard to reach populations Critical SkillsProven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict resolution skills Focus on ResultsDemonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes Alignment with Organizational ValuesCommitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission and values Indigenous UnderstandingKnowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families Interdisciplinary Team, CollaborationProven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity and cultural awareness MCC PhilosophiesStrong knowledge, commitment and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed and community development principles in all aspects of the work CommunicationExcellent verbal and written communication skills and proficiency in the use of computer systems AssetsPost-Secondary Education in a Social Services field All positions are subject to Criminal Record, Child, and Adult Abuse Registry checks.
