The RPN provides trauma-informed nursing care, including intake, assessments, and clinical procedures for HIV, gender-affirming, and PrEP services. They are also responsible for patient flow coordination and maintaining medical supplies across multiple clinic sites.
Requires current RPN registration with the CNO and 3-5 years of practical nursing experience, preferably in community primary care. Candidates must have a G2 license, reliable vehicle access, and proficiency in electronic charting.

This Registered Practical Nurse (RPN) position will work at our clinics that offer HIV care, gender affirming care, and PrEP services in Kitchener, Guelph, and occasionally Cambridge.
The RPN is a key member of an interprofessional team delivering trauma-informed, person-centred, and culturally safe care to priority populations. The role supports smooth patient flow at the clinic, performs client intake, assessments, and clinical procedures, provides thorough health education, and coordination of services and supplies across multiple locations.
Care Coordination & Patient Flow Maintain smooth, timely movement of patients through appointments across multiple clinic sites. Triage clients and incoming messages to identify urgent needs. Support follow-up systems (lab follow-up, medication renewals, client calls). Provide patient education related to HIV care, PrEP, hormone therapy, and harm reduction.
Clinic Operations Ensure adequate stock of medications, vaccines, medical supplies, and other required equipment. Follow infection prevention and control protocols, including cleaning and disinfection of clinical spaces. Support clinic setup and shutdown at different locations as scheduled. Utilize electronic medical records for accurate and timely charting, documentation, and communication.
Current Registration as a Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO). Thorough knowledge and proficiency in nursing principles, practices, and CNO standards. 3–5 years broad-based practical nursing experience; community primary care preferred. Ability to provide compassionate, non-judgmental, trauma-informed care in challenging situations. Strong communication and teamwork skills and ability to work effectively within a multi-disciplinary environment. Ability to self-manage own time and prioritize tasks. Demonstrated commitment to community-based health care and harm reduction principles. Experience with electronic charting (i.e. Telus PS Suite) and computer proficiency (i.e. Microsoft Suite programs including Outlook, Word, and Basic Excel). G2 license and reliable access to a vehicle.
