• Guelph Community Health Centre prioritizes people who face barriers to health and accessing services, and people who identify as: Homeless or at risk of homelessness Living on low income Newcomers to Canada with language barriers Having moderate to severe mental health and/or addiction issues Having moderate to severe disabilities Indigenous First Nations, Inuit and Métis Black, mixed race and racialized populations 2SLGBTQIA+ Experiencing intersectional oppression Vulnerable children and their families experiencing conflict, isolation or attachment struggles What is GCHC?
• GCHC is located in the vibrant city of Guelph, Ontario, and provides primary care services and community health programs using an interdisciplinary, team-based approach.
• We have innovative client-centered services delivered by interdisciplinary teams.
• Our clinical team consists of family physicians, nurse practitioners, and registered practical nurses, who work collaboratively with allied health workers, including social workers, dietitians, outreach workers, and cultural interpreters.
• Guelph Community Health Centre works within a determinants of health framework which emphasizes health promotion and education, disease prevention, anti-oppression, and recognition of special risk factors (social, physical, psychological, etc.) which may affect the health of clients.
