Provide one-on-one support to clients to facilitate successful community reintegration through strength-based planning and activity coordination. Collaborate with a Case Work Team to identify risk factors and document client progress.
Requires a post-secondary diploma or degree in human services or an equivalent qualification. Must possess a valid Class 5 driver's license and a current criminal record check.
Community Access Outreach Worker (CAOW) Part-Time (20 hours/week) – Full Benefits
The Government of Alberta’s Persons with Developmental Disabilities (PDD) program funds services that work in conjunction with CJHS, to help support clients with coordinating and accessing services so that they can live as independently as possible in their communities. PDD services offer a wide array of supports including, Home Living Support, Respite, Employment Support and Community Access. This program is for the implementation of the following: Services that promote an individual’s inclusion in community activities, and personal development, through activities such as arts, education, recreation, community association programs and participation in the volunteer sector.
