Health Navigator

• Community Outreach
• Identify individuals who may be experiencing barriers to accessing healthcare.
• Support vulnerable populations including Elders, individuals experiencing homelessness, and those facing addictions or mental health challenges.
• Participate in outreach activities to increase awareness of health services.

• Program Documentation & Reporting
• Maintain accurate client documentation and service records.
• Collect data for program reporting and funding requirements.
• Monitor emerging community health trends and identify service gaps.

Qualifications

Education • Diploma or Degree in one of the following preferred

• Social Work
• Nursing
• Community Health
• Indigenous Studies
• Human Services
• Combination of education and relevant experience may be considered.

• Experience
• Experience working with Indigenous communities and knowledge of Indigenous health issues.
• Experience navigating healthcare systems or providing case management support.
• Experience working with vulnerable populations including individuals facing addiction, mental health challenges, or housing instability.

• Knowledge & Skills
• Strong understanding of Indigenous culture, traditions, and community values.
• Knowledge of Alberta Health Services programs and healthcare systems.
• Understanding of trauma-informed care and culturally safe practices.
• Excellent communication and advocacy skills.
• Ability to build trust and maintain confidentiality.
• Strong organizational and case management skills.

• Additional Requirements
• Valid driver’s license and reliable transportation.
• Criminal Record Check and Vulnerable Sector Check.
• Ability to travel to regional health facilities when required.

• Working Conditions
• Combination of office work, community outreach, and travel to healthcare facilities.
• Interaction with individuals experiencing crisis, trauma, or health challenges.
