Location

Provide individual coaching, advocacy, and crisis intervention for youth experiencing or at risk of homelessness and sexual exploitation. Manage a caseload of 1 to 8 youth, assisting them with housing, basic needs, and connections to healthcare and community services.
Requires a post-secondary degree or diploma in social sciences or human services and at least 2 years of experience with vulnerable populations. Must possess a valid Class 5 driver's license, a reliable vehicle, and clear criminal record and child intervention checks.

The Role: Housing Support Worker (Case Manager- YTA)
The Case Manager works within Housing First programs for youth ages 16 to 22, who are currently experiencing, or are at risk of homelessness. With a focus on harm reduction, this scattered-site housing program aims to house youth in the community of their choice. Some of the barriers Case Managers reduce for their clients include those to housing, basic needs, medical appointments, advocating for system supports, mental health services and addiction counseling. Case Managers have a caseload ranging from 1 to 8 youths at a time.
This position reports directly to a Trellis Program Manager.
