The Follow-Up Support Worker provides intensive case management to individuals transitioning into housing through the Housing First program. Responsibilities include conducting home visits, developing individual action plans, and connecting clients to essential community resources.
Candidates must hold a post-secondary degree or diploma in Social Work or a related field and possess at least two years of experience working with vulnerable populations. A valid driver's license, reliable vehicle, and relevant certifications like First Aid and CPR are required.

The Follow-Up Support Worker (FSW) provides intensive case management services to individuals who have recently transitioned into housing through the Housing First program. This role focuses on maintaining housing stability, improving the overall quality of life, and supporting participants in achieving their personal goals. The FSW is responsible for the effective planning, implementation, and evaluation of program services. The FSW carries out their duties in a professional manner and in accordance with all YMCA policies and procedures, standards of practice, health and safety standards, child protection standards, and contractual, legal, and regulatory requirements.
