The LPN will conduct comprehensive nursing assessments, develop customized care plans, and provide direct clinical interventions such as wound care and infusion therapy. They will also collaborate with an interdisciplinary team to optimize patient outcomes and educate families on chronic disease management.
Candidates must be a Registered Licensed Practical Nurse with the BCCNM and possess at least 2 years of acute care experience. A valid driver's license, vehicle, and professional liability insurance are required to perform community-based nursing duties.
