The Nurse Practitioner will provide holistic care to patients with stable and chronic illnesses, focusing on health promotion and mental health support. They will collaborate with a multidisciplinary team to develop care plans and manage complex health needs within the community.
Candidates must hold a BScN and NP-PHC certification with current registration with the College of Nurses in Ontario (CNO). A minimum of two years of experience in mental health or a related field is preferred.
