The clinical pharmacist will provide comprehensive drug distribution and clinical pharmacy services to patients across various hospital departments. They will also collaborate with inter-professional teams, participate in patient education, and support the teaching of students.
Candidates must hold a degree from an accredited pharmacy program and possess Part A registration with the Ontario College of Pharmacists. Previous hospital experience with unit-dose and IV admixture programs is required.
April 10th, 2026 What we are looking for: As a clinical pharmacist at Oak Valley Health, you will work with the inter-professional team to provide clinical pharmacy services to Markham Stouffville Hospital (MSH), Uxbridge Hospital and Reactivation Care Centre (RCC) patients. MSH provides services to a wide range of patients in the following areas: Surgery, Medicine, ICU/CCU, Childbirth and Children, Psychiatry, Rehabilitation, Palliative, Complex Care, Oncology and Emergency. The Uxbridge Hospital provides services in Medicine, Complex Care, Emergency and Day Surgery. In this role, you will provide a range of drug distribution and clinical pharmacy services. In addition you will participate in patient education, support teaching students and provide drug information to nursing and medical staff . You will work collaboratively with the pharmacists and pharmacy technicians as an integral member of our pharmacy team. What you bring to the role: Graduated from an accredited university pharmacy program Licenced under Part A registration status with the Ontario College of Pharmacists Hospital pharmacy residency is preferred Previous experience in a hospital setting with unit-dose and IV admixture programs is required Demonstrated experience identifying, resolving and preventing drug related problems to optimize medication therapy and safety. Demonstrated computer efficiency is essential Excellent communication and interpersonal skills and the ability to work collaboratively with the members of the interdisciplinary team Evidence of professional development, continuing education and leadership experience is preferred Demonstrated good attendance and performance records with the ability to maintain these same standards Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required Work Schedule: The successful candidate will be available to work 8 hour shifts days, evenings and weekends; between the days of Monday to Sunday, and on-calls as required. Please note schedules may change due to operational needs.
