The Discharge Planner collaborates within an interdisciplinary team in a fast-paced Emergency Department setting, focusing on enabling timely patient and family transitions through the hospital system. Responsibilities include formulating, facilitating, and coordinating discharge plans based on patient needs, employing critical thinking for complex situations, and ensuring patients receive the right care in the right place at the right time.
Candidates must possess a Bachelor's degree in a Health Discipline (Master's/MSW preferred) and current registration with a Regulated College, along with 2-5 years of clinical experience in an acute care hospital setting, including essential discharge planning experience. Key requirements also involve demonstrated ability to conduct psychosocial assessments, strong interview skills, and comprehensive knowledge of community resources and relevant public hospital legislation.

Under the leadership of the Manager, Discharge Planning, the Discharge Planner will work within a fast paced environment as a collaborative team member of an interdisciplinary team. The primary goal of Discharge Planning services is to support and enable patients and families to make timely transitions through the hospital system. The Discharge Planner does all of this in an effort with the goal of providing an exceptional patient and family experience. The Discharge Planner understands the Flow and Capacity challenges of a hospital system and works to achieve that the patients are in the right place, at the right time, receiving the right care.
