Occupational Therapist – Home & Community Care

Position Summary

Our client is one of the few stand-alone, global leaders in Pediatric Health and is home to over 400+ staff and Physicians dedicated to helping children and youth live their best lives. The are currently looking for The Occupational Therapist – Home & Community Care Occupational Therapist provides occupational therapy services to patients in homes, schools and other areas as needed, through conducting clinical assessments, and developing treatment plans and goals in consultation with the patient, family and other interdisciplinary team members, in accordance with the standards of the College of Occupational Therapists of Ontario. The Occupational Therapist will support School-Based Rehabilitation Services (SBRS) Program and in-home therapy services, as needed. The Occupational Therapist may also provide some assessment and/or treatment for children from birth to age 18 in the Development & Rehabilitation department. These services may be provided virtually or at one of clients locations across Eastern Ontario.

Job Description

• Completes assessments of patients in a variety of treatment settings including private homes, and schools, and performs an analysis based on objective, subjective and functional findings.
• Establishes realistic, measurable and time limited treatment goals and develops treatment plan. Ensures that occupational therapy treatments are initiated with the informed consent of the patient or substitute decision maker in accordance with the Health Care Consent Act.
• Provides direct treatment and/or support and teaching to informal caregivers, school personnel and other health care professionals involved.
• Participates in case conferences when required and consults with other members of the health care team in fostering a holistic approach to care.
• Completes reassessments to review patient's progress and treatment goals and reports to Care Coordinator and other members of the health care team in order to ensure continuity of care and determine if other resources and referrals are required.
• Plans in collaboration with the patient, family and Care Coordinator, for the termination of service when patient's goals are achieved and/or when patient no longer requires the service.
• Completes assessments and recommendations for adaptation aids, assistive devices and equipment necessary to achieve treatment goals respecting where appropriate the requirements of the Ontario Assistive Devices Program. Completes documentation and maintains appropriate patient records in accordance with the standards of the College of Occupational Therapists of Ontario and clients policies and procedures.
