As a Registered Practical Nurse (RPN), the role involves supporting residents by administering medications, managing narcotics, applying treatments, transcribing physician orders, and collaborating with the care team. Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto.
PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga.
The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery.

Key Responsibilities: As a Registered Practical Nurse (RPN) you will support PCH residents by assuming the following responsibilities: Community Driven: Establishes and maintains good interpersonal relations with colleagues, residents, families, volunteers, and the community Caring Service: Collaborates with other members of the care team Assumes responsibility for medication administration, narcotic control, removal of discontinued, unused, or expired medication, application of treatments and dressings, transcription of Physician orders, reordering of medications, preparation and other skills Promotes the PCH's philosophy of care Promotes and supports the Resident’s Bill of Rights Completes other duties as assigned Professionalism: Is accountable to the Nursing leadership team for providing nursing care and clinical intervention for the care and services delivered to the Residents on the assigned RHA Functions in accordance with Guidelines for Ethical Behaviour in Nursing Participates in the Home Area programs (e.g.
Occupational Health and Safety Program, Quality Monitoring and Analysis Program, and Fire Safety).
