The Field Nurse Case Manager coordinates and delivers services for patient support programs, acting as a primary contact for healthcare providers to ensure patients have access to support and therapy. Responsibilities include performing patient registration, providing medical support and education, administering case-management protocols, and liaising with various healthcare parties to resolve issues.
Candidates must be a Registered Nurse in good standing with the College of Nurses in their respective province and possess a minimum of two years of clinical experience. Experience in immunology, rheumatology, or dermatology, as well as experience in Patient Support Programs, are considered strong assets.

The Field Nurse Case Manager (FNCM) is an integral part of the Patient Services team, responsible for the coordination and delivery of services related to the patient support programs offered by Sentrex Health Solution. The FNCM is responsible for managing all aspects of program support and may act as a primary point of contact to their assigned health care providers (HCP) and ensures patients are provided easy access to the support program and to their drug therapy. In addition, FNCM will assist the prescribing physician to complete patient enrolment; including on-site visits to the physician office to gather all pertinent and appropriate information to complete the necessary forms in a concise, organized and professional manner.
This is a field‑based role, with most of the time spent traveling to and working directly at clinics throughout the assigned geographic region.
