The LPN provides palliative care services, focusing on symptom management and patient/family education regarding medications and lifestyle changes to improve quality of life. This role involves close collaboration with specialists, initiating discussions on advanced directives, and following prescribed plans of care while continuously evaluating patient response.
Candidates must be a graduate of an approved LPN school of nursing and hold a current LPN license in Massachusetts, along with required CPR certification. A minimum of one year of recent experience in an acute care facility or equivalent is necessary, with homecare and computer skills being preferred.
