The Pharmacist provides pharmaceutical care by reviewing and assessing medication therapies to optimize patient outcomes. They collaborate with multidisciplinary healthcare teams across individual or multiple clinical units to minimize medication-related problems.
Candidates must be licensed or eligible for licensure by the Ontario College of Pharmacists and hold a Bachelor of Science in Pharmacy or a PharmD. Proficiency in pharmacy systems and demonstrated skills in independent practice and leadership are required, with a residency preferred.
The Pharmacist is a member of the multidisciplinary healthcare team that provides pharmaceutical care to patients at Hamilton Health Sciences. The Pharmacist reviews and assesses medication therapies and works with the patient and the healthcare team directly to optimize medication use and minimize medication-related problems to achieve desired patient outcomes. This work may be performed on individual clinical units, or may be across multiple units to support the medication system needs of the site.
