Liquid Personnel is currently recruiting for a Social Worker to join an Hospital Discharge team based in Halton.

• What will your responsibilities be?
• In this role, you will play a key part in facilitating safe and timely hospital discharges, especially during the challenging winter months.
• You will manage a complex caseload independently and efficiently, ensuring that each case receives the attention it deserves.
• Your support will be crucial for individuals and families transitioning from hospital to community care, helping them navigate this critical period with ease.
• Collaboration with multidisciplinary teams will be essential to ensure seamless, person-centred care, making sure that every individual’s needs are met comprehensively.
• This role requires a proactive and compassionate approach, dedicated to improving the lives of those you serve.

Qualifications

and Experiences: To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
