Overview
Conduct physical and mental health assessments for clients with mental illness and/or substance use disorders. Develop and implement treatment plans. Provide case management. Deliver mental health counselling and education to clients. Provide primary health care and mental health services. Provide education, leadership, and coordinate care to support clients in managing their own health. Function in a collaborative practice model as part of an interdisciplinary team to provide comprehensive primary and/or mental health care. Deliver services largely through home visits and community outreach, with occasional clinic or satellite pop-up appointments. Complete suicide risk assessments and create safety plans. Provide psychoeducation to clients, families, and support staff. Provide physical health assessments and chronic disease management. Support clients and their natural supports to achieve optimal health. Provide services related to harm reduction and/or knowledge of other services designed to meet the needs of people who use substances, including Opioid Agonist Therapy services
