Overview
Provide comprehensive bio/psycho/social services designed to optimize client recovery and functioning. Assess and treat complex clients with co-occurring disorders and serious functional impairment. Provide service coordination and urgent response as required. Utilize specialized treatment modalities/therapies for individuals, groups, and/or families. Arrange access to specialized services (substance use, forensic/criminal justice, developmental disability, acquired brain injury). Ensure transition plans are in place for clients transferring between levels of care, including admission to acute care and discharge planning/follow-up. Document assessments, treatment formulations, care plans, and progress notes. Consult and share information with community agencies and the public about mental illness and mental health/substance use services. Deliver and participate in in-service and other educational programs as required. Perform other related duties as assigned
