Deliver client and family-centered care with cultural humility using a trauma-informed and evidence-informed approach. Perform initial client assessments and provide health promotion, interventions, coaching, and teaching based on client needs and urgency. Facilitate and manage client transitions across the healthcare continuum to optimize health and support prevention of avoidable hospital admissions and emergency department presentations. Collaborate with the client, Primary Care team, and other health care services to support care planning and information flow across settings. Encourage knowledge exchange in day-to-day activities and empower clients to engage with and connect to their communities
