Overview
Provide compassionate inpatient substance use withdrawal nursing care at the Nechako Centre. Practice in accordance with professional practice standards and the BC College of Nurses and Midwives (BCCNM) code of ethics. Work under general supervision of Manager or designate and report to the Withdrawal Management Unit Program Lead or designate. Collaborate with the multidisciplinary care team and partners to deliver holistic, person-centered, recovery-oriented care. Assess and monitor clients and develop a plan of personal care and interventions with the patient/family and care team. Assist individuals in managing medical symptoms of substance abuse withdrawal. Complete initial and ongoing client assessments and provide appropriate nursing interventions. Communicate effectively with clients, families, the public, medical staff, and interdisciplinary team members. Integrate and evaluate pertinent data to problem-solve effectively. Create culturally safe, trauma-informed health care settings through individualized care planning and collaborative service provision

Conduct street outreach in encampments, adult shelters, drop-in programs, and other areas where unhoused young people gather. Engage youth using trauma-informed and harm reduction approaches while maintaining professional boundaries. Provide basic needs support (food, clothing, hygiene) and crisis stabilization in community spaces. Provide harm reduction tools such as naloxone kits. Assess youth needs and provide meals/food packages in accordance with safe food handling practices. De-escalate physically and verbally hostile situations when necessary. Provide ongoing support to youth with system navigation and crisis support. Support youth in accessing external services (e.g., housing, emergency shelter, legal identification). Assess youth for referrals to other Covenant House Toronto programs and appropriate community services. Engage with local businesses and maintain relationships within the catchment area. Support enhanced services during extreme cold and extreme heat alerts (with program leadership support). Follow harm reduction protocols, including administering naloxone when overdoses are suspected or occur. Maintain situational awareness for unpredictable/unknown environments. Maintain daily and monthly documentation on the ETO platform. Participate in supervision and team meetings. Complete fund requests as needed based on youth needs (e.g., housing changes and landlord payments). Participate in program evaluations by completing ETO and outcome tools. Submit required reports (daily, monthly, semi-annual, and year-end). Participate in internal and external committees and other assigned duties
