Overview
Provide timely, responsive, evidence-based treatment and clinical supports across the continuum of available services for clients with serious severe and persistent mental health problems and/or problematic substance use and/or development disability issues who may or may not be in crisis. Provide assessment, treatment, case management, community support, and urgent response as required. Function as a primary clinician using specialized treatment modalities/therapies for individuals, groups of clients, and/or their families. Support initial intake and assessment for clients interested in the Opiate Agonist program. Work with physicians and community services to ensure therapy initiation, support, streamlined transition, and continuity of care. Coordinate overdose prevention education. Function as part of a multidisciplinary care team (Hospitals & Communities Integrated Services or Tertiary Mental Health Services)
