Overview
Provide nursing and clinical assessments for individuals with developmental disabilities and co-existing mental health disorders. Deliver intervention and treatment within a habilitative focused model. Provide outreach and consultation services to individuals in the Vancouver Island/Gulf Island areas. Work as a member of an interdisciplinary care team within organizational frameworks and standards. Support care for severe challenging behavior and/or substance misuse alongside mental health needs. Collaborate with clients' support networks to promote rehabilitation/recovery focused care. Partner with community agencies to provide continuity of care. Apply nursing standards and continuously improve quality and safety. Communicate effectively with clients and stakeholders in writing and verbally. Travel between multiple sites and to clinics as required, including use of a personal vehicle if needed
