Overview
Provide individualization of drug therapy for oncology patients using patient charts, patient contact, and collaboration with health care providers. Develop individualized therapeutic plans and supportive care medication plans. Interact with patients and the health care team to ensure appropriate medication therapy. Develop discharge plans for inpatients and medication calendars for outpatients. Assess patient comprehension and monitor compliance with therapeutic plans. Prepare and dispense medications and pharmaceutical supplies in accordance with relevant federal and provincial legislation. Provide verbal and written drug information to physicians, nurses, other health care professionals, pharmacists, the lay public, and patients through individualized counselling. Assist the care team with provision and/or financial support of non-BCCA benefit medications to optimize therapeutic plans (e.g., compassionate release programs, emergency aid options). Provide in-service education to other health care professionals within the Agency. Provide instruction and supervision to pharmacy students and/or pharmacy residents
