Overview
Gather information relevant to participants’ neurodevelopmental concerns, needs and risks through interviewing, observation, and meetings with caregivers and service providers. Assess gathered information to identify participant problems and assessment/service needs. Develop and implement intervention plans within program guidelines. Provide support, guidance and problem-solving to participants individually or in groups related to neurodevelopmental concerns. Participate in development, modification and evaluation of participant service plans with the integrated case management team. Participate in integrated case management meetings to report participant activities and progress. Provide information on and referrals to other community service providers, resources and professionals as required. Provide neurodevelopmental skill building (e.g., parent-child interaction, child development, strengths-based supports, behaviour management). Provide emotional support and feedback to participants. Plan and conduct group educational sessions on topics related to FASD and neurodevelopment. Liaise with and promote participants’ interests with community service providers, professionals and school personnel; accompany participants to meetings/appointments as required. Maintain related records and statistics and provide reports to the supervisor. Protect and uphold confidentiality. Perform other related duties as required
