Overview
Provide direct patient care to clients of the Urgent Primary Care Centre (UPCC), Patient Medical Homes / Family Physician / Nurse Practitioner Clinics, or an Island Health Primary Care Clinic. Practice in accordance with professional standards and the BC College of Nurses and Midwives (BCCNM) code of ethics. Deliver client and family-centered care with cultural humility using a trauma-informed and evidence-informed approach. Conduct initial client assessments and provide health promotion, interventions, and health coaching/teaching based on client needs and urgency. Facilitate and manage client transitions across the healthcare continuum to optimize care and support prevention of avoidable hospital admissions and emergency department presentations. Collaborate with the client, the Primary Care team, and other health care services to support client care planning and information flow across settings. Encourage knowledge exchange in day-to-day activities and empower clients to engage with and connect within their communities
