Overview
Work independently as a member of the Primary Care Network team and provide advanced clinical social work skills and techniques. Act as a clinical resource in social work, education, and counseling for an identified population of patients with chronic diseases and/or barriers related to social determinants of health. Participate in case management. Provide assessment, care planning, intervention, and consultation when social, economic, emotional, or psychological circumstances present barriers to care or limit patients' function. Provide education and counseling using principles of adult learning. Facilitate and build effective relationships with primary care providers, other disciplines, and community partners to support a team approach to patient care. Work with multidisciplinary teams and communicate effectively with patients, families, caregivers, and the care team. Travel between multiple sites may be required and transportation arrangements may require use of a personal vehicle
