Provide social work services as a member of the interdisciplinary team to incorporate psychosocial elements of a health condition into client and family health care planning needs. Promote effective coping and adaptation to challenges brought on by illness and hospitalization to support clients’ optimal functioning and health. Assess and plan for safe return to community within the patient’s home and/or other levels of care. Perform duties according to established standards and procedures of the Social Work Department and policies of the Northern Health Authority
