Deliver interventions focused on specific skills training and support needs, including relapse prevention, community adjustment, and employment skills. Facilitate client empowerment and competency development within the four domains of psychosocial rehabilitation (personal life, education, leisure, and work). Offer rehabilitation and recovery-oriented services that respect individual strengths, interests, capacity, and choice. Support skills transfer, generalization, and maintenance to maximize client growth and independence. Monitor client goal attainment and proactively assist individuals in averting potential crises. Report client status and progress to the Case Coordinator or multidisciplinary team, including noted reactions to medication.
