Provide individualization of drug therapy using patient information to develop therapeutic plans. Assess clinical response through patient contact and collaboration with health care providers. Interact with patients and the health care team to ensure appropriate medication therapy. Develop discharge plans for inpatients and medication calendars for outpatients. Assist with formulation of therapeutic plans for supportive care medications and assess patient comprehension. Answer patient questions and assist with therapeutic plans for alternative and complementary treatments. Monitor patient compliance with therapeutic plans. Prepare and dispense medications and pharmaceutical supplies in accordance with relevant federal and provincial legislation. Provide verbal and written drug information to physicians, nurses, other health care professionals, and patients through individualized counselling. Assist with provision and/or financial support of non-BC Cancer benefit medications to optimize therapeutic plans (e.g., compassionate release programs, emergency aid).
