Overview
Independently manage a caseload and apply disability management practices to support timely return-to-work outcomes. Assess needs, identify barriers, and intervene to support recovery and work readiness. Develop and maintain comprehensive return-to-work plans based on functional abilities and job demands. Adjust plans as needed to support ongoing progress and recovery/return-to-work efforts. Make decisions informed by evidence, assessment, and applicable legislation, policy, and practice standards. Partner with internal and external stakeholders (workers, employers, care providers, vocational rehabilitation consultants) to implement evidence-based return-to-work strategies. Monitor goals, objectives, timeframes, and assignments; evaluate plan effectiveness through continuous communication. Chair regular team meetings to review progress toward objectives and goal achievements. Contribute to case management procedures and promote quality initiatives through client-centered service and continuous improvement
